Partner2Care (P2C) offers an independent advisory service that is available to support people who have a need for care at home and would like support to arrange or manage your care.
Our Nurse Assessors can visit you where you are living to help you to plan your care and support needs and develop a personalised care and support plan with you.
Our team of Payroll, Brokerage and Support Officers can also visit you where you are living to support you to set up the services you have chosen which could include the recruitment and employment of support staff, a payroll service for your employees and if you choose ongoing email and telephone support.
Our dedicated Trainer will develop and deliver a tailor made training programme for you and your support staff to equip you and your care team with the knowledge and skills you need so that you can be assured that your care is being delivered safely. We can also signpost to other health and social care providers or training providers.
Your P2C adviser/assessor will:
- Visit you at home to explain the help they can provide
- Advise you about setting up a bank account for your care needs
- Help you recruit and employ staff
- Help you to set up a system for dealing with tax and National Insurance for someone you employ
- Offer a payroll service if needed
- Help and explain how to keep financial records to show what you have spent

Meet the Team

Tracey Roberts
Tracey is a Registered General Nurse with over 35 years of experience in the Health Sector. She trained and worked at Charing Cross Hospital in London and served as a District Nurse in West and Central London, Oxfordshire, and Berkshire before relocating to the Southwest in 2000. Her career spans private healthcare, NHS Continuing Healthcare Services, and Community Services, where she worked on service improvement and redesign, including the development of Personal Health Budgets. Tracey is an ILM Business and Executive Coach and supports individuals across the health and social care sectors. At Partner2Care, she oversees clinical, training, brokerage, and payroll services that assist Personal Health Budget holders. She also helped develop ‘One off Personal Budgets’ (Discharge Support Grants) to facilitate hospital discharge or prevent hospital admission.

Daniela Hart
Daniela is a Registered Adult and Children’s Nurse with over 30 years of experience. She has a strong background in Continuing Healthcare and Personal Health Budgets and has managed all types of assessments including DST by MDT, Hospital and Fast Track Reviews, and Retrospective Reviews. As Clinical and Operational Lead at Partner2Care, Daniela also serves as an ILM Level 7 qualified coach. She is dedicated to delivering person-centred care and supporting people to have greater choice and control in managing their healthcare.

Georgina Jones
Georgina has been a Registered Nurse since 1986 and holds a District Nursing qualification. Starting in hospital settings, she transitioned to community nursing, focusing on promoting independent living through information, guidance, and training. She has extensive experience in Continuing Healthcare, working on complex assessments and facilitating end-of-life care. Georgina is passionate about empowering people to manage their own health and make informed decisions about how they live their lives.

Linda Cord
Linda is a Registered General Nurse and Mental Health Nurse with a Non-Medical Prescribing Qualification. She also holds qualifications in First Aid and Manual Handling. Linda began her nursing career in Glasgow, then worked in London and North Somerset across various sectors including hospitals, community nursing, and occupational health. As Partner2Care’s Trainer, she is dedicated to improving knowledge and encouraging learning among those who care for individuals in their homes.

Sarah Davies
Sarah is a Registered Nurse in Learning Disabilities with 23 years of experience. Since graduating from Sheffield University in 2000, she has worked with adults with profound learning disabilities, autism, and behaviours that challenge. Sarah values holistic, person-centred care. She holds a Level 4 Leadership and Management qualification and has experience as a team leader, NVQ assessor, and student nurse mentor. She also holds a City and Guilds Assessor qualification and is training in First Aid and Manual Handling instruction.

Rebecca Francis
Becky joined Partner2Care in December 2014 as the first Brokerage and Support Officer, initially working in Sales and Marketing. Her role has evolved significantly alongside the service. Skills: Makaton

Samantha Woods
Sam joined Partner2Care in October 2017 as a team Administrator and became a Brokerage and Support Officer in January 2019. She has a strong interest in finance and is one of the Payroll leads for the team.

Charlotte Pike
Charlotte joined Partner2Care in 2023 as a Brokerage and Support Officer. She previously worked with the Funded Healthcare Team in the BNSSG Integrated Care Board and the Community Discharge Coordination Centre. She has experience in catering and fundraising and is now thriving in her NHS role.
