About Partner2Care

Partner2Care (P2C) offers an independent advisory service that is available to support people who have a need for care at home and would like support to arrange or manage your care.

Our Nurse Assessors can visit you where you are living to help you to plan your care and support needs and develop a personalised care and support plan with you.

Our team of Payroll, Brokerage and Support Officers can also visit you where you are living to support you to set up the services you have chosen which could include the recruitment and employment of support staff, a payroll service for your employees and if you choose ongoing email and telephone support.

Our dedicated Trainer will develop and deliver a tailor made training programme for you and your support staff to equip you and your care team with the knowledge and skills you need so that you can be assured that your care is being delivered safely. We can also signpost to other health and social care providers or training providers.

Your P2C adviser/assessor will:

  • Visit you at home to explain the help they can provide
  • Advise you about setting up a bank account for your care needs
  • Help you recruit and employ staff
  • Help you to set up a system for dealing with tax and National Insurance for someone you employ
  • Offer a payroll service if needed
  • Help and explain how to keep financial records to show what you have spent
  • Be at the end of a phone if you need advice

Meet the Team

Tracey Roberts

Business and Service Delivery Manager

Tracey is a Registered General Nurse by background and has been in the Health Sector for over 35 years. Tracey trained and worked at Charing Cross Hospital London and as a District Nurse across West and Central London, Oxfordshire, Berkshire before moving to the Southwest in 2000.

Tracey has worked within the private healthcare sector, managed NHS Continuing Healthcare Services and across Community Services as a Service Improvement and re-design Manager where the development of Personal Health Budgets was part of her project portfolio.

As an ILM Business and Executive Coach, Tracey believes that with support, people are empowered to make the right choices for themselves and is proud to also support people across the health and social care sector through her coaching practice.

Daniela Hart

Clinical and Operational Lead

Daniela is a very experienced Registered Adult Nurse and Registered Children’s Nurse of over 30 years. She combines an in depth knowledge and experience of both acute and primary, community and continuing healthcare with a strong background in Continuing Healthcare and Personal Health Budgets. She has worked in CHC for all types of assessments and reviews, including DST by MDT; Hospital assessments, Fast Track Reviews; and Retrospectives Reviews and currently Personal Health Budgets for adults and children.

Daniela is the Clinical and Operational Lead for Partner2Care and also supports staff both in Sirona and other organisations as an ILM 7 qualified coach.

Daniela enjoys the opportunity to support people to have choice and control in how they manage their healthcare and is committed to supporting and delivering person-centred care.

Zoë Higgs

Nurse Assessor / Trainer

Zoë qualified as a registered nurse in 2008 as a mature student. After working on the wards at Southmead Hospital she moved into working in the Community for Sirona care and health.

All Zoë's past nursing roles, Stroke Rehabilitation, End of Life Care, and Community Nursing as well as being a link nurse for Continence, have given her the confidence and opportunity to develop essential clinical and holistic skills for patient’s, their relatives, and friends.

Sarah Davies

Nurse Assessor / Trainer

Sarah is a registered Nurse in Learning Disabilities with twenty-three years’ experience. After graduating from Sheffield University in September 2000, Sarah has supported adults with profound and multiple learning disabilities, autism and behaviours that challenge.

Sarah has an interest in holistic, person – centred care. She has a leadership and management level 4 qualification, has worked as a team leader and manager, an NVQ assessor and sign off mentor for student nurses. She has a City and Guilds Assessor qualification and is undertaking additional training to teach first aid and manual handling.

Rebecca Francis

Payroll, Brokerage and Support Officer

Becky was the first Brokerage and Support Officer to join the team back in December 2014. Originally employed in a Sales and Marketing capacity, Becky’s role has developed with the service. Skills: Makaton

Samantha Woods

Payroll, Brokerage and Support Officer

Sam joined the Partner2Care team in October 2017 as the team Administrator before becoming a Brokerage and Support Officer in January 2019. Sam has a keen interest in finance and is one of the Payroll leads for the team.

Charlotte Pike

Payroll, Brokerage and Support Officer

Charlotte joined the team as a Brokerage and Support Officer in 2023, having previously worked with the Funded Healthcare Team in the Bristol, North Somerset and South Gloucestershire Integrated Care Board and before that in the Community Discharge Coordination Centre. Charlotte has worked within catering as well as fundraising and is now happily settled within the NHS.
