A new service from Partner2Care

We know from what we do that people have better health and well-being outcomes when they are in control of what they want, to make their daily lives better and we have been working in partnership with individuals since 2015 to do just that.

The team at Partner2Care (P2C) have been busy looking at ways we can support people who would benefit from some of the services we already provide for people who are Continuing Care eligible and have a Personal Health Budget with their Clinical Commissioning Group, but who either self-fund their care or have a Personal Budget with their Local Authority.

What we have learnt is that some of what we offer may help people who just need some help with planning their care, to know ‘what’s out there,’ how to access support, how much it costs and to navigate their way around ‘the system’, help with finding and recruiting carers and payroll.

Earlier this year P2C set out to provide these services as a ‘pick and mix’ offer and with the agreement of Sirona care and health have a new service called P2C+. We will be running this service as a pilot to see what you need and how we can support you, and are delighted to bring our expertise to others in need of help and support.
